Do I need a confirmed procedure date?
No. You may contact us while still planning your procedure. We can discuss general availability and recovery options before your final surgery date is confirmed.
Can a physician or family member submit an inquiry?
Yes. Surgeons, care coordinators, family members, and authorized representatives may submit an inquiry. Patient authorization may be required before certain information is shared.
Is this form appropriate for a medical emergency?
